CLINICAL APPLICATIONS OF DIFFERENT BAHIRPARIMARJANA CHIKITSA

ABSTRACT
Bahirparimarjana Chikitsa comprises a group of external therapeutic procedures aimed at cleansing, pacifying, and restoring the balance of the doshas through direct action on the twacha. As the skin serves as a major interface for sensation, absorption, thermoregulation, and protection, it plays a central role in mediating the effects of these therapies. Classical texts emphasize that externally applied medicaments are absorbed through romakupa and dhamanis after undergoing transformation by Bhrajaka Pitta, thereby exerting systemic as well as local effects. This article elaborates the principles, mechanisms, indications, and clinical applications of various Bahirparimarjana modalities - including Abhyanga, Swedana, Lepa, Udvartana, Parisheka, Gandusha, Moordhni Taila, and others-integrating Ayurvedic concepts with modern physiological explanations such as nitric oxide-mediated vasodilation, VEGF-induced angiogenesis, calmodulin pathways, and heat shock protein activation. These therapies aid in Srotoshodhana, Shoola-hara, Shothahara, Vata-Kapha Shamaka, and Vrana-Ropana through enhanced microcirculation, lymphatic drainage, tissue nutrition, detoxification, and cellular repair. Bahirparimarjana Chikitsa is effective across a wide spectrum of conditions-including Kushta, Prameha, Amavata, Sandhigata Vata, Vatarakta, Vatavyadhi, and Visarpa-and serves as Purva, Pradhana, and Paschat Karma in many Panchakarma procedures. The review highlights its utility in both preventive and curative healthcare, especially where systemic therapies are contraindicated or impractical, such as in bedridden patients. The multidimensional benefits of Bahirparimarjana underscore its significance as a unique contribution of Ayurveda to integrative medicine.
