Abstract
UNDERSTANDING THE MECHANISM OF VAMANA KARMA THROUGH CLASSICAL PRINCIPLES AND MODERN EMESIS PHYSIOLOGY

ABSTRACT
Background: Vamana Karma, one of the principal purification procedures of Panchakarma, is indicated predominantly in Kapha-dominant disorders and aims at expelling vitiated doshas through therapeutic emesis. Although its clinical utility is widely acknowledged in Ayurvedic practice, the mechanistic understanding of Vamana in contemporary biomedical terms remains inadequately explored. Objective: This article aims to analyse the mechanism of Vamana Karma by integrating classical Ayurvedic principles with modern neurophysiological mechanisms involved in the emetic reflex. Methodology: Classical Ayurvedic literature describing dosha mobilization, drug properties, and Vata subtypes involved in Vamana were reviewed and correlated with current understanding of vomiting physiology, including cortical influences, chemoreceptor trigger zone activation, nucleus tractus solitarius pathways, and peripheral gastric mechanisms. Results: The emetic process in Vamana Karma can be interpreted through three principal mechanisms: cortical triggers corresponding to Prana Vata activation, systemic drug-mediated stimulation of the chemoreceptor trigger zone correlating with Vyana Vata–mediated distribution, and gastric distension along with vagal stimulation activating the nucleus tractus solitarius leading to Udana Vata–driven upward expulsion. The preparatory procedures of Snehana and swedana facilitate dosha mobilization toward the stomach, thereby enhancing the efficiency of therapeutic emesis. Conclusion: Vamana Karma demonstrates a coherent mechanistic basis when interpreted through modern emesis physiology while retaining its classical Ayurvedic framework. Integrating these perspectives provides a rational scientific explanation for therapeutic emesis and supports further clinical and experimental exploration of Panchakarma interventions.
